Client Privacy, Privilege, and Consent Notice

1. Confidentiality and Privilege

All communications between you and [Your Name/Practice Name] are protected by attorney-client privilege [or psychotherapist-patient privilege, as applicable] and the strictest standards of professional ethics. This means that information shared during our interactions—including the fact that you have contacted this office—is confidential and will not be disclosed to third parties without your express written consent, except where required by law (such as in cases of imminent harm to self or others).

2. HIPAA Compliance and "First Contact"

In accordance with the Health Insurance Portability and Accountability Act (HIPAA), our commitment to your privacy begins the moment you reach out to us.

  • Initial Inquiry: Any "Protected Health Information" (PHI) you provide during your first contact—whether via phone, encrypted messaging, or contact form—is handled with the same security protocols as a formal patient record.
  • Data Security: We utilize HIPAA-compliant storage and communication methods to ensure your preliminary data remains secure before a formal provider-client relationship is established.

3. Informed Consent

By engaging with our initial intake process, you are providing Informed Consent for the collection of necessary data to determine the suitability of our services for your needs.

  • The Scope: Consent involves your understanding of how your data is stored, the limits of confidentiality, and your right to terminate communication at any time.
  • Relationship Status: Please note that an initial inquiry or consultation does not, by itself, constitute the formal commencement of a professional relationship. A formal agreement/contract will be executed should we decide to move forward together.
